How to Apply
How do I submit an application?
All interested candidates must complete an online application found at www.lfh.org/careers.
If you do not have access to a computer, you are welcome to use the computers in the Lake Forest Hospital Human Resources department, located on the 3rd floor of the hospital at 660 Westmoreland Road, Lake Forest, IL. You are welcome to stop by anytime from 7:00am to 4:00pm, Monday through Friday. (Please note that, on the 3rd Wednesday of each month from 11:30am to 1:30pm, the computers may not be available). Please call (847) 535-6447 if you have any questions.
What will happen to my application?
Applications of qualified candidates will be electronically forwarded to the Hiring Manager for review. The Hiring Manager will then call those candidates who most closely match the requirements for the position and arrange interviews.

What should I bring to the interview?
Please come to the interview prepared to provide at least three references with verified phone numbers. References can be current or former managers, supervisors/preceptors and instructors.
How will I know if I am the selected candidate for the job?
A representative from our Human Resources department will call you and extend the formal offer.
Receiving a Position from Lake Forest Hospital
What happens after I am offered a position and I accept?
Human Resources will assist you in arranging an appointment for your pre-employment health screen with our Occupational Health Department. You will also arrange a time to complete your pre-employment paperwork in the Human Resources department. Your health screen must take place within 48 hours of your formal offer.
What will I need for the Health Screen?
You will be asked to present proof of vaccination or immunity (negative titer) for:
- Measles
- Mumps
- Rubella
- Rubeola
- Varicella
- TB—a two part test done within the past year
If you do not have these records, titers will be completed at Lake Forest Hospital’s expense. If the titers show that you lack immunity, we expect that you will be vaccinated, at Lake Forest Hospital’s expense as well. If you refuse to be vaccinated, employment will be denied.
What do I need to bring to Human Resources when I complete the pre-employment paperwork?
- Proof of identity—photo ID such as a drivers license
- Proof of eligibility to work in the United States—Social Security Card, resident alien card, Passport, birth certificate, etc.
- Voided personal check to authorize direct deposit for your first paycheck
- Tax forms will be completed at this time as well
- Original professional licenses and CPR card, if necessary for your position
Your First Day at Lake Forest Hospital
When will my first day of employment be?
You will start on a General Orientation Day, which is conducted every other Monday. You will be scheduled for the soonest orientation that you are available to attend following completion of the necessary pre-employment health screens.
How long is the general orientation?
General orientation is a full day, usually starting at 8:00am. Lunch is provided by the hospital and the final two hours of the day will be spent in your department as your department-specific orientation. Orientation beyond this day will be tailored to your individual needs based on your past experience in the position.
Where do I park on orientation day?
On your first day, please choose a spot not too close to the main entrance, leaving the closest spaces for our patients and visitors. Employee parking regulations will be explained during orientation.
What information is covered during general orientation?
Hospital policies and procedures, customer service expectations, safety and security issues, details regarding benefits. and a tour of the hospital all compose the first day of orientation for you. You will also be given computer access codes and online education instructions, and your photo will be taken for your ID badge to be created and given to you that day.
What if something happens and I cannot attend orientation?
Please call Human Resources at (847) 535-6701 to report that you will not be at orientation. You will then be contacted to reschedule your start date for the next orientation in two weeks, as you cannot begin work until you attend orientation.
